What is a PAM survey?
The Patient Activation Measure® (PAM®) is a 10 or 13 item questionnaire that assesses an individual´s knowledge, skill and confidence for managing their health and health care. The measure assesses individuals on a 0-100 scale. There are 4 levels of activation, from low (1) to high (4).

Why is patient activation important?
Findings from over 180 peer-reviewed studies (Hibbard, 2015) indicate that: Higher activated individuals are more likely to engage in positive health behaviors and to have better health outcomes. Higher activated patients tend to have better care experiences. Higher activated patients have lower costs.
What is Pam in case management?
Case managers can use the Patient Activation Measure (PAM) to assess patient populations and guide case management goals and tactics. The goal of value-based care is to keep patients healthy and prevent new chronic conditions. Activation involves helping patients improve self-management and create better habits.

When should PAM tool not be used?
PAM® is suitable for use with people with mild/moderate mental health conditions such as anxiety and depression. Feedback from the Learning Set in 2016 showed that PAM® is not suitable for those with severe mental health conditions as their capacity to engage with the survey may be impaired.
What are the stages of patient activate?
Activation appears to involve four stages: (1) believing the patient role is important, (2) having the confidence and knowledge necessary to take action, (3) actually taking action to maintain and improve one’s health, and (4) staying the course even under stress.
